Line an 8-inch square baking dish with parchment paper.

In a saucepan, combine sweetened condensed milk, unsweetened chocolate, butter, vanilla extract, and salt.

Cook over low heat, stirring constantly, until the chocolate and butter are melted and the mixture is smooth.

Remove from heat and stir in chopped nuts if desired.

Pour the mixture into the prepared baking dish and smooth the top.
